ABSTRACT

Abstract Objective: To describe the speech pattern of patients with hereditary Spastic Paraplegia type 4 (SPG4) and correlated it with their clinical data. Methods: Cross-sectional study was carried out in two university hospitals in Brazil. Two groups participated in the study: the case group (n = 28) with a confirmed genetic diagnosis for SPG4 and a control group (n = 17) matched for sex and age. The speech assessment of both groups included: speech task recording, acoustic analysis, and auditory-perceptual analysis. In addition, disease severity was assessed with the Spastic Paraplegia Rating Scale (SPRS). Results: In the auditory-perceptual analysis, 53.5% (n = 15) of individuals with SPG4 were dysarthric, with mild to moderate changes in the subsystems of phonation and articulation. On acoustic analysis, SPG4 subjects' performances were worse in measurements related to breathing (maximum phonation time) and articulation (speech rate, articulation rate). The articulation variables (speech rate, articulation rate) are related to the age of onset of the first motor symptom. Conclusion: Dysarthria in SPG4 is frequent and mild, and it did not evolve in conjunction with more advanced motor diseases. This data suggest that diagnosed patients should be screened and referred for speech therapy evaluation and those pathophysiological mechanisms of speech involvement may differ from the length-dependent degeneration of the corticospinal tract.

ABSTRACT

Abstract Cutaneous leishmaniasis is characterized by ulcers with raised edges and a granular bottom, mainly on the lower limbs. This is a case report of a male patient with an ulcer on the left plantar region. The diagnosis was confirmed by positive PCR for L. braziliensis and the presence of amastigotes of Leishmania sp. in the histopathological examination. After treatment with Glucantime, the patient showed full healing of the ulcer. The unusual location of the ulceration calls attention to atypical presentations of leishmaniasis, and the importance of histopathological examination and PCR, leading to the appropriate diagnosis and treatment.

ABSTRACT

Abstract This article presents an integral approach to work in community projects, based on Guattari's three ecologies and its dialogue with psychosocial theorists, since it involves the need to combine technological interventions with social approaches. These contributions are explored to point out the need for dialogue in the implementation of sanitation actions, mainly in the rural area and in traditional communities, involving the individual, the groups served and their territorial culture. The approach presented was implemented in a joint action with the Caiçara Community of Praia do Sono and the Forum of Traditional Communities of Angra dos Reis, Paraty and Ubatuba (FCT), based on the Observatory of Sustainable and Healthy Territories of Bocaina (OTSS / Fiocruz). It could be verified that the inclusion of the actors in the social mobilization for the sanitation can entail an effective social participation that generates both a subjective change in the conscience of the diverse local actors and structural gains that promote health and quality of life. The panorama covered shows the importance of a global understanding of the problem, but also, of a simultaneous local, territorialized action, adapted to each reality through genuine dialogue and horizontal participation.

ABSTRACT

Abstract As sanitation conditions in Brazil are precarious in rural areas, it is important to build territorial actions that value culture and strengthen good living based on solidarity economy to achieve sustainable solutions. Dardot and Laval see the "common" as a counterpoint to the current political and economic context, in which individuals assume the role of protagonists in their lives and manage the "commonalities" for their community. In this approach, the Oswaldo Cruz Foundation (Fiocruz), through the Observatory of Sustainable and Healthy Territories of Bocaina (OTSS), constituted an ecological sanitation research in the Caiçara Community of Praia do Sono, in Paraty, with the participation of the community during the whole process and, through a qualitative evaluation, the relationship between the community and the common could be understood. Local builders were hired as social mobilizers, thus valuing endogenous resources. Through this articulation, joint action with public agencies was sought to foster actions that came from the community itself and inclusive public policies that put a society at the center of the process. Supported by the concept of common and by semi-structured interviews, this psychosocial study examines the unfolding of the solidarity economy.

ABSTRACT

OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

ABSTRACT

ABSTRACT Objective: To evaluate the effects of photobiomodulation therapy in redox status, angiogenesis marker - vascular endothelial growth factor - and in the functional recovery in denervated muscle. Methods: A total of 32 female Wistar rats underwent a crush injury and were randomly divided into four groups: Light Emitting Diode Group 2 and Control Group 2 (muscle collected 2 days after injury), and Light Emitting Diode Group 21 and Control Group 21 (muscle collected 21 days afterinjury). Light Emitting Diode Group 2 and Light Emitting Diode Group 21 received two and ten light emitting diode applications (630±20nm, 9J/cm2, 300mW), respectively, and the Control Group 2 and Control Group 21 did not receive any treatment. The function was evaluated by grasping test at four moments (pre-injury, 2, 10 and 21 post-injury days). The flexor digitorum muscle was collected for analysis of immunolocalization of vascular endothelial growth factor and redox parameters. Results: Functional improvement was observed at the second and tenth post-injury day in treated groups compared to control (p<0.005). The muscle tissue of treated groups presented higher immunohistochemical expression of vascular endothelial growth factor. Photobiomodulation therapy decreased the oxidative damage to lipid in Light Emitting Diode Group 2 compared to Control Group 2 (p=0.023) in the denervated muscle. Conclusion: Photobiomodulation therapy accelerated the functional recovery, increased angiogenesis and reduced lipid peroxidation in the denervated muscle at 2 days after injury.

ABSTRACT

Sarcoidosis is characterized as a granulomatous disease that affects different human organs, especially the lungs, and its pathogenesis is little known. In this case, the patient started with nonspecific symptoms, such as weakness, weight loss, and sporadic dry cough, being hospitalized for extension of the propaedeutics. The initial diagnostic hypothesis suggested was a possible case of multiple myeloma, based on the anemia, renal dysfunction, hypercalcemia and, above all, the osteolytic lesions presented by the patient. However, the diagnosis of sarcoidosis was made after bone marrow and inguinal lymph node biopsies that showed granulomatous myelitis and lymphadenitis, respectively. The therapy instituted was based on the administration of corticosteroids and on measures to reduce the level of calcium. The patient was discharged, with clinical improvement, for outpatient follow-up of the disease. It is concluded that sarcoidosis has no curative treatment, but immunosuppressive therapy is effective in controlling the progression of the disease, giving the patient a favorable prognosis.

ABSTRACT

ABSTRACT Hereditary spastic paraplegias (HSP) are a group of genetic diseases characterized by lower limb spasticity with or without additional neurological features. Swallowing dysfunction is poorly studied in HSP and its presence can lead to significant respiratory and nutritional complications. Objectives: The aim of this study was to evaluate the frequency and clinical characteristics of dysphagia in different types of HSP. Methods: A two-center cross-sectional prevalence study was performed. Genetically confirmed HSP patients were evaluated using the Northwestern Dysphagia Patient Check Sheet and the Functional Oral Intake Scale. In addition, self-perception of dysphagia was assessed by the Eat Assessment Tool-10 and the Swallowing Disturbance Questionnaire. Results: Thirty-six patients with spastic paraplegia type 4 (SPG4), five with SPG11, four with SPG5, four with cerebrotendinous xanthomatosis (CTX), three with SPG7, and two with SPG3A were evaluated. Mild to moderate oropharyngeal dysphagia was present in 3/5 (60%) of SPG11 and 2/4 (50%) of CTX patients. A single SPG4 (2%) and a single SPG7 (33%) patient had mild oropharyngeal dysphagia. All other evaluated patients presented with normal or functional swallowing. Conclusions: Clinically significant oropharyngeal dysphagia was only present in complicated forms of HSP Patients with SPG11 and CTX had the highest risks for dysphagia, suggesting that surveillance of swallowing function should be part of the management of patients with these disorders.

ABSTRACT

Abstract: Disseminated leishmaniasis is a severe and emerging form of American tegumentary leishmaniasis. Disseminated leishmaniasis is defined by the presence of more than 10 polymorphic cutaneous lesions, distributed over more than two noncontiguous parts of the body. Nasal mucosal involvement is observed in almost half of cases. Disseminated leishmaniasis patients present with a decreased production of Th1 cytokines in the peripheral blood due to the attraction of leishmania- activated T cells to the multiple cutaneous lesions. Disseminated leishmaniasis development is poorly understood and is related to a complex network involving environmental, host immune response, and parasite factors, in which L. braziliensis polymorphism plays an important role. Disseminated leishmaniasis is a challenging disease to cure, presenting a high failure rate of 75% to pentavalent antimony therapy. Despite its importance and severity, this form of American tegumentary leishmaniasis has been poorly studied and documented, deserving greater attention from professionals working in endemic areas.

ABSTRACT

ABSTRACT Objective. The aim of this study was to evaluate AChE activity in total blood and the FRAP levels in samples from dogs with mammary tumors before and after surgery, as well as the relationship between these variables with immunohistochemical markers of tumor (E-caderina, ki-67, COX-2). Materials and methods. In this study, 13 dogs with mammary tumors were divided into two groups (A and B). The group A was formed by dogs with tumors smaller than 3 cm of diameter, and the group B was formed by dogs with tumor of 3 cm of diameter or larger. The AChE activity and FRAP levels were evaluated before and after surgery and the immunohistochemistry were performed at the tumors. Results. The AChE activity was significantly increased (p<0.05) in dogs with mammary cancer compared to control animals, and neither surgery or tumor size affected the AChE activity (p>0.05). FRAP levels before surgery were significantly lower (p<0.05) compared to control animals. Also, FRAP levels increased significantly after surgery in animals of the group A compared to data before surgery, a fact not observed in dogs from the group B. E-cadherin showed low significant positive correlation with FRAP levels (r=0.37, p-value=0.05); COX-2 showed a moderate significant positive correlation to FRAP (r=0.55, p-value<0.05); and COX-2 showed a low significant positive correlation to AChE (r=0.32, p-value=0.01). Conclusions. AChE and antioxidant levels are modified in dogs with mammary cancer. These variables are involved in various physiological functions, and thus, they might be related to disease pathogenesis.

ABSTRACT

(AU)

(This article relates the panorama of sanitation in Brazil with a focus on the reuse of organic waste and sanitary sewage, through ecological sanitation models, in order to promote health. The biodigester is an example of social technology that makes it possible to sanitize and obtain energy in the form of natural gas through anaerobic biodigestion to rural and peri-urban areas. In the study, biodigester was classified among its applicability: rural, industrial and residential. However, in the Brazilian context, residences coming from disordered occupation and rural areas generally do not have adequate sewage treatment. Thus, data were collected on different models of biodigesters, through a case study of the biodigesters systems of Vale Encantado and Águas do Imperador. A focus was given to residential biodigesters, such as social technologies, based on a case study of the Vale Encantado community. For the construction of this study, the methodology was: Bibliographic Review, Documentary Review, Qualitative Research, Case Study and Participant Observation with field output. I t can be demonstrated that it is possible to solve in a decentralized way the final disposal of organic waste and domestic sewage, for 20 people, with application in a community restaurant (Vale Encantado), and for approximately 3,000 people, applicable in a community or peri-urban area (Waters of the Emperor). It is concluded that biodigesters are a social technology alternative for sanitation, especially in regions where conventional solutions are not a financially viable possibility. (AU)

ABSTRACT

Abstract Objective. Due to the increased parasitic resistance, there is a need to explore alternative insecticides, including natural products, such essential oils. In this sense, the aim of this study was to evaluate, for the first time, the insecticidal and repellent effect of Cinnamomun zeylanicum (free and nanoemulsion forms) in vitro and in vivo. Materials and methods. For that, the insecticidal effect of the free form was tested at 1.0, 5.0 and 10%, while the nanoemulsion form was tested at 0.5, 1.0 and 5.0% against adult house flies, Musca domestica. For validation of the test, the flies were sprayed with the diluent (water and triton 10%) and with blank nanoemulsion (without C. zeylanicum essential oil). Moreover, in vivo test was performed using cows naturally infested with Haemotobia irritans. Results. The results demonstrated that cinnamon oil (10%) and the nanoemulsion (5%) were 100% effective against M. domestica after 90 minutes of exposure. The repellent effect was tested in vivo using 5% of cinnamon oil on Holstein cows naturally infested by Haemotobia irritans. The flies were counted at 0, 1, 2, 3, 9 and 24 h after cinnamon oil treatment. It was verified significant decrease (p<0.05) in the number of flies on cows sprayed with cinnamon in all times. Conclusions. Based on these results, C. zeylanicum showed insecticidal effect (in vitro) against domestic flies and repellent effect (in vivo) against the horn fly.

ABSTRACT

This study evalueted the prevalence of Staphylococcus aureus, Streptococcus agalactiae and Escherichia coli in milk samples from 257 goats (513 half-udders) and ten bulk tanks, from ten dairy goat farms of São Paulo State, Brazil, by multiplex-PCR. The samples were screened by microbiological culture (gold-standard), and tested by different multiplex-PCR protocols for the detection of each bacterium. A total of 178 half-udders resulted positive by microbiological culture, with coagulase-negative staphylococci (70%), S. aureus (13.5%), S. intermedius (7.9%), and Enterobacteriaceae (4%) the prevalent pathogens. In other way, multiplex-PCR detected 173 pathogens in 151/523 (28.9%; CI95% 25.2-32.9%) milk samples 144/513 (28.1%) half-udders and 7/10 (70%) bulk tanks, with E. coli (86/162, 51.9%) and S. aureus (50/162, 30.9%) the prevalent ones in half-udders, and S. aureus (6/10, 60%) and E. coli (4/5, 36.4%) in bulk tanks. Multiplex-PCR showed a high performance for the detection of three bacteria at a time in mastitic goat milk direct from half-udders or bulk tanks. Thus, this multiplex-PCR protocol proved to be an adequate tool for the identification of the most common mastitis pathogens, independent of their phenotypic characteristics in the diagnosis of clinical mastitis in goats, allowing a continuous and better vigilance and monitoring the herd, being included in quality programs.(AU)

ABSTRACT

ABSTRACT Objective. The aim of this study was to evaluate the influence of subcutaneous supplementation with diphenyl diselenide ((PhSe)2) in dairy sheep infected with gastrointestinal nematodes on animal health and possible damage to environment when the feces of these animals will be used for fertilizing. Materials and methods. The experiment was performed using 16 primipara dairy sheep, that were divided into two groups: the group A as control and the group B supplemented with 3 µmol/kg of (PhSe)2 subcutaneously. Blood samples were used to determine the hepatic function, as well as the protein and lipid metabolism in animals. Feces were used to determine the number of helminths eggs per gram of feces (EPG), as well as used for ecotoxicology tests. Results. The (PhSe)2 supplementation not affected the helminths reproduction, since the EPG did not differ (p>0.05) between groups. Total protein and globulin levels increase (p<0.05) in supplemented animals, while the seric alanine aminotransferase (ALT) levels decrease (p<0.05) in the end of experimental design. Cholesterol levels increase (p<0.05) in the supplemented animals, while triglycerides, albumin and urea not differ between groups (p>0.05). The feces of supplemented sheep not interfered the springtails reproduction. Conclusions. At the administered dose, the (PhSe)2 is not able to control the parasitism, however, it did increase the globulins and cholesterol levels, that are important to immune response and for sheep reproduction, respectively. Also, the feces of supplemented animals with (PhSe)2 can be used as organic fertilizing, without negative impacts to environment.

ABSTRACT

Using a retrospective study, 493 cats tested for FeLV and FIV were selected for analysis of the association between hematologic findings and positivity at immunoassay test. Individual and hematologic variables were assessed considering the influence of results using univariate and multivariate logistic regression analysis. Out 153 of the 493 cats were positive for FeLV (31%), 50 were positive for FIV (10.1%) and 22 were positive for both FIV and FeLV (4.4%). Multivariate analysis detected significant associations between FeLV infection and age below 1 year (p=0.01), age from 1 to 10 years (p=0.03), and crossbreed (p=0.04). Male cats were more likely to be FIV-positive (p=0.002). Regarding hematological changes, FeLV-positive cats have higher odds to anemia, leukopenia and lymphopenia than FeLV-negative cats. FIV-positive cats are more likely to have anemia than negative. Identification of associated factors related to animal status and correlation of hematological disorders with infection by retroviruses in cats could be useful for detecting these retroviral diseases in cats.(AU)

ABSTRACT

Behavioral problems have importance not only in animal welfare and the quality of human-animal relationship, but also in public health. Behavior problems are a major reason of abandonment and subsequently the fate of these animals are shelters or even euthanasia. Furthermore, the aggressiveness is a risk factor for zoonoses transmission. In order to assess the frequency of factors related to behavior problems, information from 229 cats were collected through a questionnaire applied to tutors awaiting for care in clinics in Veterinary Hospital of the Federal University of Rio Grande do Sul (HCV-UFRGS). Among the behaviors scratch furniture was the most common problem (65.1% - CI95%: 58.5 to 71.4%), followed by aggressiveness (61.3% - CI95%: 54.2 to 67, 1%), inappropriate elimination (37.1% - 9 CI95%: 31.3 to 42.1%) and excessive vocalization (23.6% - CI95%: 20.1 to 29.8%). Data were analyzed by logistic regression with a logit link function. Univariate analysis identified the presence of scratching as significant protective factor, that may contribute to the reduction of scratching furniture (Relative risk [RR] = 0.31- CI95%: 0:19 to 0:53). Females and frightened cats were identified as having more difficulty were for friendly relationship with other cats RR = 3.56 (CI95%: 1.78-7.11), RR = 2.84 (95% CI: 1.60-5.04) respectively; also the higher the number of cats in a residence, the greater the chance for inappropriate elimination. Neutered cats before puberty had less chance of developing inappropriate elimination RR = 0:43 (CI95%: 0.21-0.88), and finally, excessive vocalization was related to obtain tutor attention RR = 2.62 (CI95%: 1:54 to 4:45). The results of this study may help prevent behavioral problems in domestic cats.(AU)

ABSTRACT

Colibacillosis is the main infectious cause of total carcass condemnation in broilers in southern Brazil. This study aims to determine the degree of agreement between the total carcass condemnation for colibacillosis in broilers slaughtered in establishments under Federal Inspection Service (SIF) with the pathological and bacteriological diagnosis. The study was conducted with 45 broilers totally condemned by colibacillosis (case) and theirs 45 respective controls (chickens without lesions). All broilers condemned had gross lesions and the controls had not. The Kappa-Cohen's test showed that these two variables had almost perfect agreement. Broilers condemned showed lesions in liver (27/45); liver and air sacs (11/45); liver and heart (2/45); liver, heart and air sacs (2/45); liver, air sacs and oviduct (1/45); liver, air sacs, heart and subcutaneous (1/45); and liver, air sacs, oviduct and spleen (1/45). There is almost perfect agreement between carcass condemnation and liver damage. Histologically, in 41 cases and 12 controls were observed lesions, the most frequent diagnoses were random necrotizing hepatitis, fibrinous-heterophilic bronchitis, acute pericarditis and lymphoplasmacytic tracheitis. In hepatitis cases was isolated Escherichia coli, Enterococcus sp. and Streptococcus sp. (10/38) and in bronchitis or bronchopneumonia E. coli and coagulase positive Staphylococcus (9/14). The polymerse chain reaction (PCR) and immunohistochemistry (IHC) tests for Mycoplasma gallisepticum and M. synoviae were negative. In cases of total carcass condemnation by Colibacillosis the liver was the main organ affected. Therefore, the condemnation criteria should be revised, suggesting conviction for hepatitis in these cases, because other bacteria can cause hepatitis, as demonstrated in this study.(AU)

ABSTRACT

ABSTRACT Objective. This study aimed to evaluate the occurrence of gastrointestinal protozoa in dairy calves and to identify potential risk factors for this type of infection. Materials and methods. For this purpose, 243 fecal samples were collected from calves up to 60 days of age in 43 dairy farms located in the West region of Santa Catarina state, Brazil. Samples were examined by centrifugal-flotation technique. Results. As a result, Giardia was present in 26.75% (65/243) of all samples, Eimeria in 21.81% (53/243), and Cryptosporidium in 20.99% (51/243). Additionally, 46.50% (113/243) of the samples were negative for any protozoa, while 39.10% (95/243) and 14.40% (35/208) showed single and mixed infections, respectively. There was a higher association between Cryptosporidium and Giardia (6.99%) in cases of mixed infections. However, the triple protozoa association had the lowest prevalence in mixed infections (2.06%). Epidemiologically, a questionnaire was applied to determine risk factors for these parasitic infections. Based on the statistical model applied, some risk factors for Cryptosporidium infections were identified, highlighting feeding management, period of time that calves stayed with their mothers (cows), and contact with dogs; the risk of contracting Giardia increased according to the milk source, while the floor type bedding, and age were appointed as risks factors for Eimeria. Conclusions. Therefore, it is possible to confirm that Giardia, Cryptosporidium and Eimeria may infect dairy calves, and the knowledge of some risk factors associated to their infection in calves.

ABSTRACT

ABSTRACT Objective. The aim of this study was to evaluate the performance, thermoregulatory characteristics and the leukogram of growing finishing pigs reared in different stocking rates kept on compact floor pens or with shallow pool pens. Material and methods. Thirty-six pigs were used in two steps (total of 72 animals were used). In the first step it was analysed performance pig (daily feed intake, weight gain and feed: gain ratio) and backfat thickness (P2). In the second step, the same analyzes were repeated, in addition, analysis of leukogram and skin temperature was performed in P2 and anal region, at the end of growing and finishing phase. Results. It was evaluated for differences between treatments and stocking. It was not found effect of floor type on growing phase, but the use of shallow pool had a negative effect on the gain and feed: gain ratio in the finishing phase, as well as lower surface temperature in the anal region on treatment with shallow pool in the finishing phase. No effect of treatments and type of floor on pig leukogram. Allowing more space/animal decreased backfat thickness. The use of shallow pool affected negatively the performance in the finishing phase. Conclusions. The shallow pool uses impair the finishing pig performance, despite improving thermolysis, with no effect observed on cell immune response.

ABSTRACT

Partner Homicide is the adopted term to the homicide gesture between people within a current or former relationship. There is not any methodical survey of Conjugal Homicide in Brazil, making journalistic sources the most reliable data. This article aims a discussion on Conjugal Homicide taking reported cases in two widely spread newspapers (O Estado de São Paulo e Diário Catarinense) in a ten-year span (2000-2010). A total of 34 cases in Greater Florianópolis and 110 cases in Greater São Paulo were found. When comparing the registered cases, there was a greater proportional incidence in the Greater Florianópolis. It was concluded by the surveyed cases that were a prevalence of male homicide gesture; precursor symptons; couple's split impact and the presence of intimate terrorism or domestic violence
